Patients experiencing headache, jaw, and neck symptoms can schedule an appointment to see Steve Kraus PT. 

Steve Kraus' office is in-network with most insurance plans. Out of network insurance plans are also accepted. Medicare, med pay (motor vehicle coverage) and workers compensation insurances are accepted. A cash discount is offered if using an insurance plan is not an option. 

Patients will need a referral from a dentist, oral surgeon or a physician in order to be treated. The referral can be brought by the patient to their first visit or the doctor may fax the referral to the office of Steve Kraus. Doctors can use their own referral or may use the attached referral by clicking here.

In addition, you may type-in the form fields first before printing it out, but the "Physician's Signature" can only be signed after it is printed out. If faxing the form, send to 404-261-0617.
