Course Registration

A Workshop for Physical Therapists and Physical Therapy Assistants.
Dentists and Physicians may also attend.

Expand your client population by gaining the academic and clinical skills needed in the evaluation and treatment of the TMD patient. The best treatment outcomes for the TMD patient population will be achieved by the physical therapist and dentist working together.

The physical examination used to diagnose and treat temporomandibular and masticatory muscle disorders will be discussed and practiced in lab. Cervical Spine influences on the masticatory system and facial pain will be reviewed.

This is a 15-hour course equivalent to 1.5 CEUs.

Registration

• Registration is accepted as soon as a course is posted on this web site.
• Course Tuition - $395
• Class size is limited. Registrations are accepted on a first-come, first-serve basis.
• Registration is accepted up to the day the course starts.
• Participants are encouraged to register for the course as soon as possible so as to avoid the course from being cancelled due to lack of course registrants.
• Only the submitted online registration form will be accepted.
• PayPal and Checks accepted as payment.
• If paying by check, it must be received within 7 days of registering for the course otherwise you are not considered a registered particpant and your spot for attending the course will not be held.
• Checks are to be made out for the full tuition to Horizon Physical Therapy LLC. (Horizon Physical Therapy is the sponsor for Steve Kraus)
• Confirmation by email will be sent to you indicating that Steve Kraus and Horizon Physical Therapy have received your registration form and payment (whether by PayPal or Check). 
• You must acknowledge that you have read the Cancellation Policy Statement in order to complete the registration form.
